Alternative Evidence for IAL2 Identity Proofing
To verify an identity at IAL2, an individual is required to complete and return an original, notarized identity verification form. If a passport, REAL ID, or Enhanced ID is presented to the notary and used to complete the form, additional evidence is not required. If one of these three items is not used, the subject whose identity is being verified must submit additional evidence to complete identity verification.
In this case, two of the following are required:
(1) A copy of a utility bill (gas, electric, water, cable TV or internet, etc.) indicating the subject's name and home address
(2) A mobile phone number billed to subject (the subject should provide this on the identity verification form)
(3) An individual NPI in subject's name (if they are a provider; the subject should provide this on the identity verification form)
(4) A bank or credit card statement from a US-based financial institution indicating the subject's name and home address
(5) An additional, original notarized identity verification form completed using an ID card issued to subject by a federal, state, or local government agency or entity, provided it contains a photograph (completed using a different piece of evidence than the first notarized form).
An annotation on the document(s) or alongside it/them should indicate that: "this document is submitted as proof of my identity and as an appointed representative of [certificate applicant organization name]", along with the subject's signature.
